  • FIT4MOM AGREEMENT

    This contract made between the client and FIT4MOM OKC METRO is a month to month agreement, and may be terminated by either party with the proper advance notice. This plan allows you to attend unlimited Stroller Strides/Barre/360 classes. One month minimum commitment is required.

    AUTHORIZATION FOR PAYMENT
    I authorize FIT4MOM OKC METRO to initiate a charge to the credit card number entered into Front Desk for my dues. This authorization is to remain effective until I have cancelled my plan through the FIT4MOM owner.

    I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with Stroller Strides to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a return fee of $25. Plan holders are responsible for updating their card information online. Plan holders may not attend class who have unpaid balances. If a Monthly Plan remains unpaid it will be cancelled.

    CANCELLATION POLICY
    To cancel this monthly plan, the plan holder must contact the FIT4MOM franchise owner 3 days prior to their next payment due date to request a cancellation. No refunds will be given for dues already paid. This plan is not transferable to another person or anywhere outside the OKC METRO franchise. Cancellation requests must be emailed to okcmetro@fit4mom.com

    HOLD POLICY
    Memberships may be placed on hold due to pregnancy leave, extended vacations, or medical injuries. Plans may be held a maximum of 4 months per hold period per one year. Hold starts will coincide with the next billing date and can end on any day of the month. When a hold period is ended, the monthly auto payments will resume on the return date. Hold requests must be emailed to okcmetro@fit4mom.com 3 days prior to their next payment due date.

    AGREEMENT
    By purchasing this plan, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
